HACC-PYP Linkages Program

Our experienced case managers work with you to understand your unique needs, help you achieve your individual goals, and connect you with available services and supports. We coordinate care, including navigating systems like the NDIS, while promoting your health, independence, and wellbeing through ongoing support, advocacy, and service coordination. What we offer

• Referrals to allied health services such as physiotherapy, occupational therapy, podiatry, dietetics and more.
• Referral to council services for Home and Community Care (HACC) assessments
• Support to enhance and sustain everyday skills for independent living
• Referral to domestic and personal care assistance services
• Nursing support
• Opportunities to connect with the community, reduce isolation, and improve wellbeing

You may be eligible if you:

• Are an adult under 65 years of age (or under 50 if you are Aboriginal or Torres Strait Islander)
• Experience psychosocial functional impairment due to disability or chronic illness
• Are not receiving support through the NDIS
• Need short- to medium-term assistance to help stay at home
