Am I eligible for the program?
You may be eligible if you:
- are aged 65 years or older, or 50 years or older if you are an Aboriginal or Torres Strait Islander person
- live in the community (not in aged care)
- are experiencing mild to moderate changes in your strength, balance, mobility or ability to do everyday activities.
You may also be eligible if you've experienced unintentional weight loss, had a fall, or had an unplanned hospital visit in the past 12 months.
Can I join if I have Support at Home or CHSP funding?
You are not eligible if you are currently receiving Support at Home services. If you have been approved for CHSP but are not yet receiving CHSP services, you may still be eligible.
What support could help me?
You can access a range of local services to support your health and wellbeing. This may include support with:
- physical activity
- nutrition
- medication management
- social connection
You'll also work with a dedicated coordinator who will get to know you, understand your goals, and help create a personalised plan that suits your needs.
How much does the program cost?
The program is free.
Do I need a referral, or can someone help me join?
No. You can refer yourself directly to the program.
Health professionals, hospitals, pharmacists, community organisations, support services, family members and carers can also make a referral.
If you'd like support with the process, a family member, carer or support person can help you contact Each, complete a referral, or support you throughout the program.

You can also speak with your GP or another trusted health professional about whether the program may be right for you.
What happens after I join?
A program coordinator will contact you to learn about your health, wellbeing and goals.
Together, you'll create a personalised plan that outlines the support that may help you achieve your goals. Your coordinator will then connect you with services that meet your needs and support you throughout the program.
How long does the program go for?
You can receive up to six months of tailored health and wellbeing support.
Where is the program available?
Each offers the program across Maroondah, Knox, Yarra Ranges, Boroondara, Manningham, Whitehorse and Monash.
Support may be provided at local centres, in your home or via telehealth, depending on your needs and preferences.
What if I already see an allied health professional?
You may still be able to join.
The program works alongside your existing care. Your coordinator can work with you and your healthcare team to make sure the support complements any services you already receive.
Will the program affect my current treatments or medications?
No. The program is designed to work alongside your current treatments.
Support may be available to help you manage your medications or arrange a medication review with a qualified pharmacist. Any decisions about medications remain with your prescribing healthcare professional.
What if I start the program and decide it's not right for me?
Participation is voluntary. If you decide the program is not right for you, you can stop at any time.
What happens after the program finishes?
Your coordinator will help you plan your next steps. This may include connecting you with community supports or other services to help you maintain your health, wellbeing and independence.
